One Medical Passport

Welcome to the Physicians Surgery Center!

Before your surgery, we ask that you complete “A Medical Passport,” an online registration form that allows you to provide complete, accurate medical information to your surgical team so we can provide an optimal patient-care experience. You may also receive a phone call, email, or text from One Medical Passport directing you to complete your medical history.

How to Complete Your Medical Passport

Use this link to be connected with One Medical Passport. Then follow the instructions below, depending on if you’re a first-time user or a returning user of the medical passport.

First Time Users Only:
Click here to create a medical passport. Follow the prompts and set up your account’s username and password. Answer the questions on each page, then click ‘Save & Continue’ on each page. Once complete, click ‘Finish’ to submit your health history.

Returning Users Only:
If you have previously created a Medical Passport & are having another procedure, you will need to verify and/or update your information. Click on this link. Then enter your previous username & password in the ‘Welcome Back’ area. Click ‘Sign In’ and then locate the purple button to update your health history.

Need Help?
On the top right hand corner of each page, there’s a Help link you may click on for assistance. If you are unable to complete your medical history online, a pre-admission nurse from our facility will contact you by phone close to the date of your procedure to complete your history with you.

Please note that questions regarding arrival or procedure time, billing/co-pays, pre-procedure instructions or medical concerns/test results must be directed to your physician's office or the facility directly.
